C.A.R.E. PLAN - Continued Assessment and Relief Enrollment Plan (Telehealth)

For a reasonable fee, you are allowed 4 monthly telehealth visits with Dr. Bain. 

Weekly visits may not be necessary every month, but are available to you if you need them.  Whether the visits are through video (doxy.me) or a phone call, you will receive the medical attention when you need it. 

4 MONTH PLAN: $508

6 MONTH PLAN: $762

12 MONTH PLAN: $1524

CARE Services include:

• Access to Dr. Bain by Phone, Secure Text or Video (up to 20 minutes)
• Same Day or Next Day Appointments
• 4 monthly visits (use them or lose them)
• Easy self scheduling online
• IN-OFFICE visits available for an additional $20 per visit (due at check in)
• Coordination of Care including orders for prescription, labs, imaging or referrals to specialists
• Interest free monthly payments ($127/month) available if needed

We do not bill insurance for our C.A.R.E. Plan services so there are no deductibles or co-pays relating to your visits.

You only pay the annual fee.

Should you need prescription medication, imaging, lab work or emergency services, you will still use your regular insurance to cover all services outside of our practice. Co Pays and deductibles may apply to services outside of our practice. 

We strive to keep our practice simple and affordable. Many insurance plans are discontinuing telehealth coverage for some services. For patients' without insurance coverage, we have constructed a simple plan. Whether you wish to see Dr. Bain in the office or via telehealth, we have options for you. 

IN OFFICE DIRECT PAY FEES: 

NEW PATIENT OFFICE VISIT  - $227 Initial Visit

ESTABLISHED PATIENT OFFICE VISIT - $127 Per visit

TELEHEALTH DIRECT PAY FEES : 

NEW PATIENT TELEHEALTH VISIT - $127 Initial Visit

ESTABLISED PATIENT TELEHEALTH VISIT - $100 Per visit
